Position Summary

Supports Tampa Bay Health Alliance population health efforts by coordinating the care of patients, conducting needs assessments of patients, and managing the case coordination process.

Position Requirements

Education: Minimum high school diploma. Currently licensed as a RN in the State of Florida. Certification in Basic Life Support (BLS) required and must be maintained. Advanced Cardiac Life Support (ACLS) required.

Experience: Minimum of 1 year of experience working wiht coordinating the clinical care of patients referring to clinical programs.

Skills: Knowledge of nursing principles, practices and techniques. Ability to communicate effectively verbally and written. Able to work under physical and mental stress. Ability to participate as a member of multi-disciplinary team in the planning of treatment for individual patients. Ability to identify problems related to the patient’s illness in conjunction with therapeutic plan, and suggests plans for resolution.

Preferred Requirements: Experience working in a managed care enviornment (ACO, Health Plan, etc) in the coordination of care of members/patients.

Requirements reflect the minimum level necessary to be eligible for this position. Equivalent combinations of education and experience may be considered.
Level 1 Background check will be completed.

Work Schedule Summary

Specific Duties and Responsibilities

Percentage 20

Assist TBHA groups in identifying and referring patients to eligible programs within TBHA groups and the patient’s insurer’s programs such as Diabetes/Nutrition, Personal Health, Complex Case Management, etc.

Percentage 30

Coordinate the activities of care managers within TBHA groups.

Percentage 20

Evaluate the effectiveness, necessity and efficiency of patient treatment plans and communicate with PCP and care delivery team to make recommendations.

Percentage 30

Educates self and TBHA group’s care managers on critical elements associated with the TBHA tarteged populations and best practices.
